Terms and Conditions

Advanced Natural Health is a ProActive Naturopathic clinic, which provides nutritional and naturopathic solutions, hands on body treatments, health coaching and advanced recovery techniques.

The undersigned individual (the Member), hereby indicates their desire to become a patient, client or member of Advanced Natural Health, pursuant to the terms and conditions of this agreement.

Health Attestation
The Member attests that they are fully able to participate in a treatment regimen of our design or choosing without undue risk.

Membership Dues
The Member agrees to pay all dues. These dues may be paid by automatic debit, bank deposit or credit card transaction, all in advance of treatment.

Weekly Direct Debits are automatically renewing every week. Weekly Direct Debits are non-refundable and are not transferable between individuals and CANNOT be extended unless you are injured, in which case we will request a medical certificate. Extensions due to medical reasons must be requested before the expiry date.

Payment will be automatically deducted from your nominated account/card each week (on your chosen day). If payment defaults you will be charged a default fee of $15.

Your agreement is scheduled for 26 weeks to allow you to book appointments 4 weeks in advance. This will be paid in weekly instalments. Your agreement will automatically renew after this time.

This information serves as a liability release, pursuant to which the Member agrees to indemnify and hold Advance Natural Health harmless against any and all claims of loss or damage without limitation.

Personal Agreement
I agree that Advanced Natural Health is in no way responsible for the safekeeping of my personal belongings while I attend my appointments.  I understand that I voluntarily participate with full knowledge that there is risk of personal injury, property loss or death. I agree that neither I, my heirs, assigns or legal representatives will sue or make any other claims of any kind whatsoever against Advanced Natural Health or its members & staff for any personal injury, property damage/loss, or wrongful death, whether caused by negligence or otherwise.

Advanced Natural Health does not assume responsibility for any lost or stolen personal property. If you do become the victim of a theft, immediately report it to a Advanced Natural Health staff member.

Rules & Participation
The Member hereby agrees to abide by all posted safety guidelines and regulations while using Advanced Natural Healths facilities and equipment.

Additionally, the Member agrees to dress and conduct themselves in a manner deemed appropriate for a health facility.

The Member shall not consume drugs, alcohol, or tobacco products on Advanced Natural Healths property.
The Member agrees not to photograph or videotape on Advanced Natural Healths property.
Advanced Natural Health reserves the right to revoke the Member’s access if these terms are violated.

Cancellation of Contract
This membership may be cancelled by providing written notice to Advanced Natural Health to email address: comealive@advancednaturalhealth.com.au. If the contract is cancelled prior to set contract expiry date – 26 week contract from signing date, the Member agrees to pay a cancellation fee of 50% of the remaining contract.

If the Member must cancel this gym membership contract due to military service or relocation to an area more than 40km from Advanced Natural Health facility, the Member may cancel without paying the cancellation fee.

Member Suspension
Members of Advanced Natural Health are available to suspend their contract for 4 weeks per 26 week contract. Suspension must be taken in 1 month blocks and communicated via email to: comealive@advancednaturalhealth.com.au. Medical Certificates are required for additional suspension.

Communicable Diseases
If you have contracted a communicable disease, please ensure you have clearance in writing from your doctor before attending your next appointment. This is for the safety and well-being of all our clients, staff and customers.

Bookings, Attendance & Payment
You will be contacted via text, phone or email should an appointment need to be cancelled. Please be aware appointments are on a first come first serve basis. To avoid disappointment, appointments can be booked in advance.

Payment must be made upon booking, NO exceptions.

Owners and/or Staff reserve the right to restrict clients to a particular treatment type for safety reasons; and Owners and/or Trainers reserve the right to turn clients away if they do not have the correct medical clearance.

Advanced Natural Health is a busy clinic. If for some reason you need to reschedule your appointment, I require 48 hours notice; otherwise you will be billed for the missed appointment. This, in consideration for others, allows me to schedule in patients from my waiting list.

Staff are subject to change without notice.

Advanced Natural Health reserves the right to cancel appointments if necessary.

If you do not cancel or reschedule your appointment outside of the 48 hour time frame or are a no-show/absent from your class you will lose your appointment.

Payment Policy
Payment must be made prior to your treatment. To book into an appointment you must either have credits in your account, or pay at the time of booking.

Binding Law
This contract shall be considered binding upon the Member and Advanced Natural Health, and shall be upheld and enforced in accordance with the laws in Queensland. Any legal proceedings related to this membership contract shall take place in courts located in Brisbane, Queensland.

The below electronically signed party & Advanced Natural Health hereby enter into this membership contract with one another in acknowledgement and acceptance of the terms listed above. By booking into your first treatment you automatically acknowledge and agree to the above Terms & Conditions.
